The Medication component is used for comprehensive support of the medication process, from the drug order through to the documentation of the administration or dispensing of the drug or the printing of the prescription for the patient.

Note that the scope of the Medication functions available is different in dynpro and smart UI. The following figure shows the interaction and prerequisites:

Medication is divided into the following areas:

  • Drug management

  • Medication orders

  • Medication events (administration, dispensing, print prescription)

To enable an overview of the patients' drug therapy from anywhere in your institution, Medication provides functions for mobile use.

The Medication functions in dynpro are available in both the inpatient and outpatient environment. The Medication functions in smart UI are available in the inpatient environment.

The medication graph of i.s.h.med charting (Charting) in dynpro provides a graphical overview of medication orders and administration events. Functions for planning the duration of orders and individual processing of orders can be called directly from the medication graph. For more information, see:

In the patient record of smart UI, you can call functions to edit events in dynpro from the medication order overview in the UIBB Orders. The medication graph of Smart Chart provides a graphical overview of medication orders and administration events in smart UI and the possibility to edit orders in dynpro. The Edit Day's Events dialog box in smart UI enables you to plan the duration of individual medication orders and change administration events. For cross-patient worklists for the administration and preparation of drugs, you use the views in the clinical work station in dynpro. For more information, see:

Implementation Considerations

To be able to enter and manage Medication Orders (orders) and Medication Events (events), you must first define drug masterdata in the system.

You can assign this drug master data to a Level of the Drug Catalog. You can define one or more levels for the relevant drug catalog, which you can use to map a drug hierarchy and, depending on your definition, can be used as levels for the drug search, the ordering, administration, and dispensing of drugs or for prescription printing. You can use the following levels:

  • Generic level (brand name and package size are irrelevant)

  • Intermediate definable level (e.g. generic drug with package information, brand drugs)

  • Brand drugs with package information

The use of different levels enables the use of the correct degree of detail for a drug at the correct time.

For example, a physician in an outpatient clinic sometimes wishes to prescribe a specific brand drug in a certain package size, another time, however, he prescribes the relevant active agent in a specific package size.

This means it can be significant, that, for example, generic drugs (see Generic Drug Definition) in various package sizes and brand drugs can be used in the same situation.

Furthermore the drug hierarchy enables simple navigation between the drug master data of the various levels in drug management.

When managing drugs in the system, there are numerous possibilities for storing detailed values (e.g. treatment duration, individual dose, determination of agents, etc.). These values are then used in the drug search to preset when ordering or administering drugs. The drugs of the various levels can therefore contain varying degrees of value details.

However, you can also work with only one level. If you only wish to use one level it will always be the brand drugs with package information level.

When creating medication orders the user has various search options in the drug master (e.g. by generic name, agents, etc.) and it is also possible to use order templates.

You can implement complex in-house workflows using flexible status profiles for medication orders and medication events. The system supports these in-house workflows with corresponding view types in the clinical work station.

In drug administration it is also possible to document the preparation of compounds (infusions, ointments, powders), determine the flow rates of infusions, document deviating administrations, etc..

In addition to the administration documentation it is also possible to document the dispensing of drugs and the printing of prescriptions.

Integration

As well as the complete integration in Materials Management (SAP MM) and Controlling (SAP IS-H CO) the module is also integrated into the functionality of SAP Patient Management (IS-H) and the clinical system (i.s.h.med). In addition to the integration in Materials Management, separate pick and fill lists are also available for the administration and issue of drugs by the pharmacy.
