Tab at Item Level
Depending on the order types in use, the clinical order can contain tab pages for the order items. These item level tab pages are used to enter information on the order items.
Note
In the order type, the system administrator defines which tab pages should be available to you when creating a clinical order, their descriptions, and the content (area) of each tab page.
Furthermore system administration can use the Business Add-In (BAdI) Control Dialogs for Header Components
to define, whether the system should display header components in expanded or collapsed form. If you already selected an item, e.g. call from a view type, when the clinical order was called, the system will display the selected item in expanded form and will not evaluate the parameter. Otherwise the system will only evaluate the parameter. Note that an implementation of this BAdI overrides the user parameter N1CORD_POS as well as the standard logic for displaying appointment, template, appointments or movements.
System administration can use the BAdI Preset New Order Items
to customer-specifically preset the following information in new order items

Preregistration Indicator
Case
Note that it is not possible to preset the item status, as this is set by the initial status according to the status profile when an item is created.

The system can flag the item component tab pages of a clinical order with LEDs (icons) as follows:
Green LED: Indicates that all entry fields required for a status change are filled
Red LED: Indicates that not all entry fields required for a status change are filled
Note
The system will only display these indicators if the system administrator has activated them either using the system parameter N1COICON
or using the user parameter of the same name.

In general, the following content can be available on tab pages for entering information:
The Admission Data
area provides fields for entering the admission data or visit data for the order items.
These entries are used to support the organization of planned admitting care units/outpatient clinics: The Preregistrations View Type shows the care unit/outpatient clinic all patients whose admission is planned, for example for the next day, when querying by admission date.
If you enter the planned admission date, the system will not create an admission, i.e. it does not create a case. Nevertheless, the system displays patients that you have preregistered for an admission in the arrivals list of IS-H care unit management.
The system sets the Private Patient
indicator using the treatment category
, if the entered treatment category is assigned to a case classification which indicates private patients in the implementation guide.
The Referral/Transfer
area provides the fields for entering the referral or transfer information on the order item of the clinical order.
Note
If you enter the Business Partner
value and a physician as the initiator
in the Order
area of the order header and the Referral/Transfer
area is available, the system will copy this physician as the family physician and the referring physician in the Referral/Transfer
area of the item level tab page.
The Transport Order
area provides the fields for entering a transport order for the order item of the clinical order.
If you create an initial transport, the system will preset the Pick Up From
field with the initiating organizational unit (OU), the Bring To
field with the treatment OU.
If, in connection with the Pick Up From
and Bring To
fields, you enter rooms, the input help for the room information contains the rooms primarily and secondarily assigned to the organizational units. Siehe Zuordnung von Organisationseinheiten und baul. Einheiten pflegen
The system administrator can use the OU parameter N1FDOEFA
to define that this Transport Service OU
should be preset.
If you have already assigned an appointment for the order item, you can no longer change the attributes of the transport order in the clinical order. You must make the changes in transport order management.
Note
When allocating an appointment for a clinical order with the Transport Orders application component the system will also create a transport order with an appointment reference for provisional patients.

The Services
area provides the fields for entering the services which should be ordered or preregistered in the order item of the clinical order.
Note
Unlike the service request, a service is not obligatory in the clinical order. This means that you can also create a clinical order without a service.
The Date
and Time
fields of the appointment, with which the service is connected, are ready for input, if the service in the same row is plannable and planning is intended due to the Appointment Allocation indicator, as well as all other checks.
If both fields are ready for input, their behavior is as follows:
If you enter an appointment (date and/or time), then this only applies to this service. As a result the system creates a separate appointment for each entry of this type made, even if the date and time is identical.
If you delete an appointment, the system behaves in the same way as when you delete the appointment displayed in the order item. The system displays a dialog box where you can decide whether you really wish to delete the appointment. If you confirm this, the system cancels the appointment.
If the removed appointment is identical to the appointment which the system displays in the order item, the next valid appointment or an empty appointment field is displayed (if the deleted appointment was the last of the item).
If you change an appointment, the system behaves in the same way as when you delete an appointment.
Note
In the case of surgery items, the desired date and appointment are never ready for input in the service component, as all services of surgeries must have the same appointment and the item can therefore only be planned using the planning tools or the clinical work station.
The selected services from the order types selection list are already preset. You can enhance these.
Note
If the localization in the service master of a service is flagged as obligatory on the Other
tab page, the system checks whether a localization was entered for this service when it is entered.
The following functions are also available as pushbuttons:

If you select this function, the system calls the appointment editor, as with the order item, for one or more selected services, as follows:
If no appointment exists, the system calls the appointment editor in create mode (Create Appointment
function).
If an appointment exists, the system calls the appointment editor in change mode (Change Appointment
function).
Note the following:
If you do not select a service, or none of the selected services are plannable, the system displays an error message.
If you have selected at least one service, the system connects the appointment which you are planning using the appointment editor, with the selected services. Prerequisite is that all selected services are plannable. If this is not the case for at least one service, the system will display an error message and cancels processing.
If on or more of the selected services already has one or more appointment, the system behaves in the same way as the Appointment Entry
function of the clinical work station. This means that the system connects all services with the new appointment. If existing appointments without services remain, the system will cancel these without a dialog box. The same applies if there are already appointments for services of an item and you plan a new appointment for the entire item. In this case the system cancels the appointments of the individual services.
If you select exactly one service with an appointment, the system will not plan a new appointment, but changes the existing appointment.
If none of the selected services has an appointment, all services receive the new joint appointment.
Note
After planning, the system may update the appointment field of the order item.
(Cycle
)
At item level of the clinical order, choose the
(Cycle
) function in the drop-down menu. As soon as you have selected the Cycle characteristic in the drop-down menu, the following tab pages are available:

This function is available to you if you have activated the business function Clinical System Situation-Based Clinical Documentation
(ISHMED_SCD
).
Note
If you make changes in the cyclically ordered services, these are only transferred into the newly generated services, but not take into account in the existing services.
The Ordered Services tab page displays all services which are ordered within a clinical order. The following functions are also available as pushbuttons:
(Cycle
)
If you have not yet defined a cycle, you can use this function to select a cycle. If you have already determined a cycle, you can use this function to access cycle definition to change the cycle. Furthermore you can cancel the cycle in the cycle definition
(Cancel Cycle
). Note that when you cancel a cycle, the system also cancel the cyclically ordered service.
(End Cycle
)
You can end a cycle with this function. If you select this function, no further individual services are generated in the future for the ordered service. The system sets the end date to the system date. The system also cancels all plannable services. Note that it is only possible to end a cyclically ordered service if at least one cyclical individual service has already been performed.
(Generate
)
You can use this function to multiply an ordered cyclical service (= ordered service with a cycle) according to the cycle definition. Note that the system can only generate once for a time area (time area = system time at time of generation + generation horizon).
On the Individual Services
tab page the system displays the services which result due to the cycle and should also be performed by the service performer.
Note
Note that in generated cyclical services the times calculated by the cycle are displayed in the fields Desired Date/Desired Time.
(Manually Add Cyclical Individual Services
)
When you call this function an individual service is displayed for the ordered cyclical service. Note that if several cyclically ordered services exist, you must determine which service an individual service should be inserted for. Note that the system can only generate once for a time area (time area = system time at time of generation + generation horizon).
(Display All Services
) or
(Display Plannable Services
)
You can use this pushbutton to choose to display all services or only plannable services. A medical service is considered to be plannable service if:
the service has status Unconfirmed Requested (UA), Confirmed Requested (BA) and is not connected to an appointment.
the service has status Unconfirmed Requested (UA), Confirmed Requested (BA), Scheduled (DI) and is connected to an appointment.

With the OU parameter N1COSRVSH
you can control whether all services or only plannable services should be displayed in the clinical order. See Organizational Unit-Related Parameters for Clinical Orders
(Sort Services
)
The following sort criteria are possible:
Chronological: The system sorts using the service time.
By ordered service: Sorting is according to ordered service.
Note that the system displays the canceled and replaced services at the end of the sort list, regardless of the sort order defined. You can use the OU parameter N1COSRVSO
to control how the services should be sorted. See Organizational Unit-Related Parameters for Clinical Orders
Visit
(Create Visit
)
If you select a service in the service component and choose
Visit
(Create Visit
), you can create a visit for exactly this service (with or without an appointment) using the Clinical Process Builder (transaction NV2000
).
Note the following system behavior:
If an appointment already exists, the system assumes that all other services which are connected with the appointment, also belong to this visit.
If you call this function with more than one selected service, calling is only possible if none of the services have an appointment (in this case the system connects all services with the visit to be created) or all services have the same appointment.
If you do not select a service and choose this pushbutton, the system displays an error message.
It is not possible to call a planning tool, since this is also possible from the order item and, as a rule, all services of an order item are listed for scheduling.
The desired date is always the same for all services of an item, as this is connected with the order item. It is still contained in the column selection, so that you can simply compare it with the appointment and movement data.
Note the following special features:
In the collective mode of the clinical order, the desired date and appointment are not ready for input in the service component.
When you copy a service in the service component (
(Duplicate Row
)) the system does not copy appointments.
For technical reasons it is necessary for the system to check all services of an item when planning an appointment for one or more services. This can lead to you not being able to plan an appointment for a service, because, when calling the planning tool, the system determined that a required entry for another service has not been filled.
Services of a surgery item must always be connected with the same appointment and the same movement. It is not therefore possible to change appointment and movement data in the service component for surgery items.
If errors occur which concern the appointment displayed in the item, the system navigates to the appointment field of the order item, otherwise to the appointment field of the service component.
The Surgery Indicator
area provides the fields for entering the indicators for a surgery (e.g. Anesthetist Required) for the order items of the clinical order.
The Surgery Team
area provides the fields for entering the surgery team for the order items of the clinical order.
You can also enter, for example, the operating surgeon in the preregistered order items, if this is already known in advance.
The Radiology
area provides the fields for entering the contraindications and radiological patient data required for radiological examinations.
The risk factors for a patient and the service master data determine the behavior of the input dialog box for radiological examinations.
Note
When a specific status change occurs for the order item, a method is activated in the system, which creates the radiology documents (EXAM and STUDY document). This status change for the creation of the radiological documents is determined by system administration.
The Transport Type
area provides the field for entering the transport type for the order items of the clinical order.
The Order Item
area provides the fields for entering the order item row of a clinical order.
