In the Dose field of the compressed order view you can use simplified dose definition to determine the daily dose of a medication order, i.e. a string of characters with individual doses, separated by commas.

Example Example

You wish to administer the following dose of a drug 3 times daily and must enter the following character string as the simplified dose definition:

  • 1 tablet: 1-1-1

  • 100 ml of an infusion: 100-100-100

  • 500 ml of a nutritional solution: 500-500-500

  • 5 ml of a compound you have mixed yourself (e.g. cough syrup): 5-5-5

  • The system will automatically store the corresponding times using the defined standard administration times of the ordering organizational unit (OU).

    Example Example

    You have stored the times 8:00, 14:00 and 20:00 for your OU. By making the following entries you can also define the administration at this time:

    • 1-1-1 for tablets

    • 100-100-100 for infusions

    • 500-500-500 for nutritional solutions

    • 5-5-5 for compounds

  • You can determine deviating times by entering a time in parentheses.

    Example Example

    The last administration should take place at 19:00 and not at 20:00. You define the following administration sequence:

    • 1-1-1(19:00) for tablets

    • 100-100-100 (19:00) for infusions

    • 500-500-500 (19:00) for nutritional solutions

    • 5-5-5 for compounds

  • You can determine a dose which cannot be defined at the time of the order for an administration time which can be defined using the value "X". The system will generate medication events for these times with the planned quantity "0". You can manually supplement the planned quantity for each individual event on the Events tab page of the detail area in the order dialog, as long as the event has not been documented as "administered".

    Example Example

    You plan a drug administration for the times 8:00 and 23:30 but can only determine which dose should be administered at these times shortly before (e.g. due to a lab parameter). The OU has a regular medication round at 8:00 but the other time in the evening is different from the normal medication rounds. You define the following administration sequence:

    • X-0-0-X (23:30)

    Note Note

    Note that it is only possible to enter the value "X" in dose definition in orders which have a daily cycle stored.

    If you use the value "X" as the individual dose in dose definition and the drug can be dosed precisely, the Check Condition indicator must be set.

    If the dose definition contains the value "X", the main dose calculator (compressed view of order) is not available in the selection help or using the pushbutton.

    Note that the Quick Administration function is not available for medication events with drugs which can be dosed precisely and with an undefined planned dose.

To determine the frequency with which this constant drug administration should take place, you can store a day cycle in the medication order. A day cycle determines only the day-specific intervals (e.g. daily, every 2 days, once a week, etc.).

Note Note

You should note that you cannot change a day cycle in the order dialog, as the dose and times are exclusively determined by the entry in the Dose field.

The system will automatically preset the day cycle using the system parameter N1MESICY.

Simplified dose definition is not possible with:

  • execution

  • continuous IV

  • continuous single administration

  • in case of immediate dose the does definition must contains a different dose than 0 or x.

Note that simplified dose definition is also possible with conventional cycles.

When a dose for standard drugs is entered the system will still check whether the drug can be divided. If necessary, the system will round the entered dose sequence up or down.

Note that the system will never round down to zero. The system always rounds up to the next dose which can be divided by the maximum denominator.

The system takes into account whether the drug can be crushed by not rounding down and informing the user that the drug can be crushed.

Example Example

If you enter 0.2 (tablets) and a maximum denominator of 4 the system will automatically round up to 0.25 (tablets). If the tablet can be crushed the dose of 0.2 (tablets) is retained.

You can also determine individual doses (e.g. 1 tablet, 100ml infusion, 1 self-made capsule, etc.) and select any cycle (e.g. "every 8 hours").

You can also create entries using the * wildcard. Note that you can only use a wildcard if you do not enter any other letters. The times are output for the ordering nursing OU.

You create a medication order.

Once you have selected the drug in the drug search or manually entered the drug name in the Mnemonic field of the compressed view of the order dialog, the system will preset the standard individual dose stored in the drug master in the Dose field, in connection with the standard times stored for the ordering nursing OU.

Example Example

You set a default standard individual dose. The standard times are 8:00, 14:00, and 20:00. The system will preset the character string as follows for the following standard individual doses:

  • 1 tablet: 1-0-0

  • 100 ml infusion: 100-0-0

  • 500 ml of a nutritional solution: 500-0-0

  • 5 ml compound: 5-0-0

In the case of standard drugs and compounds, depending on your user-specific presettings the system will display entered numbers which are not whole numbers as decimal numbers, or as fractions (see User-Specific Presettings). This applies only to dimensional dose units, dosages with a dose unit of a dimension are always display as decimal places.

Example Example

You have selected fraction display in your user-specific presettings. If you enter 1.5 – 1 – 0.5 and confirm, the system will display 1½ - 1 – ½ .

In case of infusions, which do not have a dose unequal to “X“ or “0“ entered, the system will not calculate a rate duration based on volume. In this case you can freely specify the duration of rates. The chart will display these durations.

Simplified dose definition is also possible for drugs which cannot be dosed precisely (e.g. creams). The system sets the value 1 for each administration time to be observed.

You define administration as 1-2-2. The system will automatically reset the dose definition to 1-1-1.

You can also use simplified dose definition within the framework of a sliding dose. The same rules apply as for simplified dose definition in the Dose field of the compressed view of the order dialog.

If you replace a drug (e.g. using “Change Drug“, “Change Administration Route“, “Change to Administration“ etc.), the system will adapt the dose to suit the new drug, if this satisfies the standard substitution rules.

Example Example

You replace standard drug A containing agent A with a dosage of 4 mg with drug B containing agent A with a dose of 8 mg. You have defined 1.5-1-0.5 as the simplified dose definition. The maximum denominator of drug B is defined as 2. The system will adjust the dosage and automatically round up to 1-0.5-0.5. Change the dose if necessary.
