Propose Alternatives
The system displays the drugs which may be used as substitutes.
The search for alternative drugs, which displays alternative drugs when a drug is changed or the change function is used in an order, groups these search results in the following different groups:
Aut idem
: The Aut idem
group contains drugs in which the following properties are identical with the drug to be replaced:
Agents
Agent Equivalent
Ratio Agent: Equivalent
Dosage Form
Administration Route
When an alternative drug is transferred into a subsequent order, the system automatically calculates the required dose for this drug.
Aut idem
(enhanced): Often the system cannot find a drug which satisfies al conditions so the system also displays additional drugs in the Aut idem
(enhanced) group, which have the same agent or the same agent equivalent, but where the other parameters differ from those of the drug to be replaced.
When transferring an alternative drug into a subsequent order, the system calculates no dose proposal for drugs from the Aut idem
(Enhanced) group, the differing agent ratios, dosage forms and adminustration routes could meant that as other dosages may be necessary here, which cannot be determined by calculation.
The following rules apply to mixed drugs (infusions, compounds):
You can only replace mixed drugs with a substitution if they are premade drugs which are administered as they are premade. You cannot change the composition. Agent relations must be the same in both drugs.
Note
You can customer-specifically override the rules for determining alternatives using the SEARCH_ALTERNATIVE
method of Business Add-In ISHMED_ME_DRUG_SUBST
.
See Customer-Specific Enhancements for Medication
Note
When transferring an anamnesis medication the first alternative drug is not automatically pre-selected. This drug is only automatically preselected once the anamnesis drug is also available in the in-house list.
Depending on the status of the order the system will change the existing order (if already saved) or create a subsequent order (for active orders).
Caution
If you select a different drug from the input help in an order which has not yet been saved, the previously selected drug will be completely replaced and the original order is discarded. This does not count as a substitution.
As soon as an order is saved, you can only replace the original drug with a substitution. This is the case even if you select the drug from the input help of drug names.
In the Select Alternatives
screen the system displays the possible drugs for the substitution. In the tree display, the system displays the original drug of the order (on a blue background) and the alternative drugs for the substitution.
The drug search is also available in the last row of this list (hotspot). In the drug search you can search for other alternative drugs which, for example, do not have the same type of drug category.
You can select the displayed fields and save them as a layout.
For each original drug you can only select and transfer one alternative drug into the order. The system replaces the original drug in the order with the selected alternative drug.
Note
If you replace a drug which has the Check Condition
indicator set (in the drug master) with a drug where a check condition is not mandatory, the indicator is once again ready for input, but not automatically reset.
Note
In the Select Alternatives
screen the system displays the active agents of the drug to be replaced (as information for the drug search), (for example, for the substitution of a multiple agent drug with two single agent drugs).
