Cause Analysis This process supports you when analyzing and documenting the causes that led to an event. The relationships between the causes are illustrated using a cause hierarchy. A cause hierarchy is a tree structure that contains a series of primary cause categories (such as technical factors or personnel factors) on which other cause categories and causes or other influencing factors depend hierarchically and with an increasing level of detail.
For a cause analysis, you can use the integration option with the industrial hygiene and safety measure object. This means you can assign a safety measure to a cause and display it in the safety measure screen.

Build Cause Hierarchy
, you must have put together the cause hierarchy you want to use for the incident/accident log. An example exists in the standard system. For more information about building a cause hierarchy, see the documentation for the IMG activity.
If safety measures are necessary, you must have created these for the respective incident/accident log entry (see Creating Industrial Hygiene and Safety Measures ).
You have opened the required incident/accident log entry and are on the
Investigation
tab page. The description of the process flow below refers to the different areas on this tab page. The following figure therefore provides a brief introduction to these areas:

You expand the required area of the cause hierarchy and select the relevant causes.
If you have assigned icons to the various node types when you configured the system, these icons are displayed in front of the causes. This makes it easier, for example, to locate root causes quickly.
For causes that require active preventive or corrective safety measures, you open the menu for

Safety Measures
in the pushbutton area and choose one of the following options:
Display Safety Measure
: If safety measures exist, the safety measure screen is opened and the safety measures are displayed.
Assign Safety Measure
: A selection list appears in which you can select a safety measure. When you have selected a safety measure, the system then displays the key and description of this safety measure on the
Investigation
tab page.
Note
You can assign a safety measure to only one cause in the cause hierarchy.
Undo Safety Measure Assignment
: Deletes an existing assignment and removes the safety measure from the assigned safety measures displayed.
You enter your final investigation result. The field has a long-text function, that is you can enter a user-defined text of any length.
