Employees can view and change their benefits plans in a portable document format (PDF) using the following enrollment options:
New Hire Enrollment: Print Version
Life Event Enrollment: Print Version
Open Enrollment: Print Version
Beneficiary Enrollment: Print Version
Employees use this application to print their enrollment form in a PDF, which gives details of the benefits plans in which they are enrolled and the plans for which they are eligible. Employees can change their enrollment plan details and submit the new details to their employer. This application displays the plan details in a form, which are based on the Open Enrollment process or on any of the configured adjustment reasons.
