 EEO Questionnaire (Example)

Your company is seeking a new employee and you have created the requisition Chief Buyer for Pharmaceutical Industry for this purpose . You are legally required to satisfy race and gender reporting requirements about the candidates to comply with the EEO ( Equal Employment Opportunity ) regulations.

You want to record the required data using an EEO questionnaire . You want to integrate the EEO questionnaire in the application wizard menu to enable candidates to complete it as part of their application materials.

You want to report on the data you collect using EEO Reporting.

The following process takes place:

  • You create the questions and the questionnaire

  • You integrate the questionnaire in the application wizard menu

  • The candidates complete the questionnaire

  • You perform EEO Reporting for all candidates

Creating Questions and Questionnaires

You create a new questionnaire of the category Equal Employment Opportunities Information. You enter the title EEO Data .

Create questions according to the legal requirements. You define the response type single choice or multiple choice . You determine a reporting area for each question in which the fields relevant for a certain type of EEO reporting are predefined . These specifications are stored in Customizing for E-Recruiting. Assign the relevant field to a question. In this way, the possible responses are automatically transferred to the responses to the question.

You draft an introduction for the questionnaire . In the introduction, you explain the purpose of the questionnaire and how to complete it. You view the finished questionnaire in the Preview and then release it . It is now available for use.

Integrating a Questionnaire in the Application Wizard Menu

To display the questionnaire in the Application Wizard , you must integrate the questionnaire in a process template , which you then assign to a requisition .

You create a new process template and assign the activity type EEO Questionnaire in Application Form to it

Assign your questionnaire to the activity type.

When you have created the process template, assign it to your requisition . The questionnaire is displayed for the candidates in the application assistant as the menu entry EEO Data , providing you have released the requisition request.

Completing the Questionnaire

Candidates apply for the job posting associated with your requisition. They find a link to the questionnaire in the menu of the application wizard which leads them through the application process. The candidates read the instructions , complete the questionnaire, and save it . Once completed, the questionnaire cannot be changed by the candidates. They can view their entries in the application overview at any time.

Performing EEO Reporting for All Candidates

Since you are authorized to perform EEO Reporting, choose the Reporting function and select the EEO report you require. A list of all EEO data is displayed, sorted by requisition and assigned candidates.
