
Printing COBRA Enrollment and Confirmation Forms
Use
You follow this procedure to print the following for one or more beneficiaries:
For example, you may want to print a replacement enrollment form or an enrollment form for an open or a life event enrollment during COBRA participation.

Enrollment forms printed for open enrollment or event-based enrollments always list all the COBRA-relevant plans in which a similarly-situated, active employee is eligible to participate, and not just those COBRA-relevant plans in which a beneficiary is was enrolled prior to the qualifying event (as you may have requested for initial offers).
The plans listed on a replacement enrollment form (printed before the qualified beneficiary has made a COBRA election) depend on your settings for initial offers of COBRA continuation.
Prerequisites
You have set up form templates in the Benefits Administration Implementation Guide in the section COBRA ® Form Setup.
Procedure
For enrollment forms that you are printing for an open or event-based enrollment, you need to enter the date as of which changes to participation will be effective.
For the selection, you can use either personnel numbers, the criteria in the General benefits information area, or the selection buttons at the top of the screen.
.A list of employees who are participating/eligible to participate in COBRA or whose dependents are participating/eligible is displayed. Expand the tree structure to see the individual beneficiaries.
. To print forms, select the appropriate beneficiaries and choose
Forms.Result
The forms are printed. Beneficiaries for whom forms are successfully printed are marked
. If errors occur, choose
to display the error log. To display more information about an error from the log, double-click on the message.
Forms with SAPscript Template
The print request is sent to the SAP System spool to be printed on the printer you specified.
Forms with Office Template
The forms are printed on the default printer set on the PC on which you are running the current session.
