This procedure describes how you create a general questionnaire and assign catalog entries to it.

For information on how to create a specific questionnaire from a general questionnaire, see Assigning Questionnaires to Work Areas.

Creating Questionnaires

...

       1.      From the Industrial Hygiene and Safety menu, choose Work Areas ® Edit Work Areas.

This takes you to the Edit Work Area: Initial Screen.

       2.      Choose Environment ® Questionnaire ® Edit Questionnaire.

The editing screen for questionnaires appears (Change "Questionnaire" View).

       3.      Choose This graphic is explained in the accompanying text.

The system creates a questionnaire and assigns a questionnaire number, which you can change. The questionnaire is given the status New - not saved.

       4.      Save your entries.

The system queries whether you want to assign the questionnaire to subject areas. Assign the questionnaire to any subject areas you think are relevant. See also Creating Subject Areas.

       5.      Enter a validity period for the questionnaire.

Assigning Catalog Entries

       6.      Choose This graphic is explained in the accompanying text Insert catalog entries to assign questions, texts, and headers to the questionnaire.

       7.      When you have completely finished editing the questionnaire, choose This graphic is explained in the accompanying text to set it to Active and save.

Caution

Once you have activated the questionnaire, you cannot make any changes other than adding catalog entries.
