Assigning Questionnaires to Health
Surveillance Protocols 
You assign one or more questionnaires to a health surveillance protocol so that the persons required to undergo that protocol automatically receive the correct questionnaire to complete.
The protocols and any questionnaires assigned to them are in turn assigned to the persons who are required to undergo health surveillance protocols because of certain factors, such as exposure to certain agents at the workplace (see Assigning Health Surveillance Protocols Manually to Persons).
· You have created a health surveillance protocol and a questionnaire.
· The protocol to which you want to assign a questionnaire has the status In Process (see Status of Health Surveillance Protocols).
· The questionnaire that you want to assign to a protocol must have the status Active (see Status of General Questionnaires).

1. In the Occupational Health menu, choose Basic Data ® Health Surveillance Protocols ® Edit Health Surveillance Protocols.
A table containing the existing health surveillance protocols appears.
2. Double-click the protocol to which you want to assign one or more questionnaires.
The detail screen for the health surveillance protocol appears.

Choose
Questionn.
The Assign Questionnaire dialog box appears.
4. Choose Yes if you want to assign questionnaires to the protocol.
The Questionnaire Assignment: Change screen appears.
5. Enter the questionnaires that you want to assign to the protocol.
The system proposes a default validity period for the protocol assignment. You can overwrite this if necessary.

The validity period for the protocol assignment must be within the validity period for the questionnaire.
6. Save your entries.
