       1.      In the planning cockpit, select a medical service.

       2.      Choose This graphic is explained in the accompanying text Invitation with the quick info text Print Invitation/Questionnaire.

       3.      In the dialog box that appears, confirm whether you also want to print out the questionnaire at the same time.

       4.      Enter the print parameters as required.

       5.      Choose This graphic is explained in the accompanying text Print.
