Questionnaires in Activities 
You can use questionnaires in CRM Enterprise and CRM Mobile to gather additional information on a product or to document customer preferences regarding a product. This product can either be your own product or a competitor product.
You are launching a new brand of soft drink. Therefore, while making sales calls at your customers’ site, you can use questionnaires to record the customers’ opinion regarding existing brands of soft drinks.

To
be able to use a questionnaire, you must have created a business activity for
a transaction type. Additionally, you must have created an
activity journal
template of the
type Questionnaire for the same transaction
type.
You
create questionnaires in CRM Enterprise using the
Survey Tool and
download them to CRM Mobile. For more information on questionnaires in
